Intrahepatic Intraductal Papillary Cystic Neoplasm of the Bile Duct: A Case Report

Purpose: Intraductal papillary neoplasm of the bile duct (IPNB) is a rare tumour with very low incidence in Western world, characterized by high risk malignant transformation and unknown prognosis. It new entity which was adopted WHO 2010 as precursor lesion cholangiocarcinoma. Intrahepatic most common site origin for IPNB. We present case an asymptomatic 63- year- old man, complex cystic on left hepatic lobe proved to be type Method: A 63-year-old Caucasian man presented our unit after routine abdominal ultrasound revealed multilocal liver lobe. The patient has previous medical history metabolic syndrome diabetes mellitus 2, hypertension, steatosis, chronic obstructive pulmonary disease. Further screening modalities (CT, MRI Abdo) showed segments ΙΙ/IVa, dimensions 4,7x4,5x3,3 cm. had intrahepatic peripheral location multilocular, grape-like multi- appearance, no apparent communication biliary tree. Iv contrast images gradual progressive enhancement multiple internal septa. radiologic features were not specific. Results: underwent hepatectomy uneventful recovery. Histopathology IPNB, completely resected (R0). follow up 2 years post-operation signs recurrence. Conclusion: diagnosis management IPNB remain challenging. multimodality imaging approach essential order diagnose assess extent, well plan optimal treatment strategy. Complete surgical resection (R0) close postoperative follow-up offers long-term survival.
